Address and Phone
This page will display address and phone information that we have on file for the
applicant. You can change any information that is incorrect or needs to be added.
You must have a mailing address on file with us. The home address is optional. If
you need to delete the home address then select the checkbox that is above the home
address. This will delete that particular address from your records. You can also
enter a home or other type of phone, and an e-mail address. Once you are done, then
click the "Next" button.
Household Members
This page will display the members that are currently part of your household. You
can remove people from your household if you would like. To the right of the person
listed, there should be a remove button for everyone except for the payee (or guardian).
You can remove people from your household that way. Newly added people will have
a "Change" button select "Change" to update the information for that person. If
you need to add people to your household, then select "Yes" to the question at the
bottom that asks if anyone else lives in your home. This will take you to a screen
where you can add people, or restore people that have been previously removed from
your household.
New or Change Household Members
If you are adding a person, please fill in the information and click next. If you
do not want to provide a SSN online, You can select the box stating that you prefer
not to declare a SSN. If you don't provide one, This person may not be included
in the TANF, Food Benefits or Medical benefits until a Social Security Number or
proof of application for a Social Security Number has been provided.
If you are adding an Undocumented Alien or prefer not to say, then this person may
not be included in the TANF, Food Benefits, Child Care benefit or Medical benefit
until Citizenship Status has been declared and/or verified.
If you are changing a person, the information we currently have will show. Change
what you need to and then click 'Next'.
New or Change Household Members
If you are adding a person, please fill in the information and click next. If you
do not want to provide a SSN online, You can select the box stating that you prefer

in the TANF, Food Benefits or Medical benefits until a Social Security Number or
proof of application for a Social Security Number has been provided.

Person in Long Term Care Facility
You will see this page if someone is currently in a Long Term Care Facility. Select
whether they are still in a facility. If they are not, you must answer when the
person left and where he/she now resides.
People Relationships
This page will allow you to set the relationships between people.
You will see a persons icon and name on the left, with the other person on the right.
Between them is a drop down list of relations. Select the appropriate one and click
'Next'.
Pregnancy
This page will allow you to select any people that are pregnant. Only female members
aged 10 and over will be in the list. If a member is pregnant, select the expected
number of births and estimated date of delivery.
Permanently Disabled
For SNAP food benefits, a permanently disabled person is someone who receives one
or more of these:
- Social Security disability payments or Supplemental Security Income (SSI) disability
payments;
- 100% rated VA disability pension;
- a disability retirement pension from a government agency
For SoonerCare (Medicaid), a permanently disabled person is someone who receives
Supplemental Security Income (SSI) or Social Security disability payments.
Household Summary
This page will give you a summary of all the people in the case. If you need to
make a change, select change from the bottom and alter the information.
Employment
This page will give you a summary of household members and their employment, if
they have any. If you need to add an employer to the household member, do so by
clicking 'Add' to the right of the household member. If you need to make a change,
select change to the right of the person and alter the employment information.
New Employment
This page will allow you to add a new employer.
Change Employment
This page will allow you to change any kind of employment information. If you wish
to delete a job check the box that says 'Check here to delete this job'. You must
also provide the date the employment ended, put that in the end date text box. (mm/dd/yyyy)
Self Employment
This page will allow you to add new self employer.
New Self Employment
This page will give you a summary of household members and their self employment,
if they have any. If you need to add any self employment to the household member,
do so by clicking 'Add' to the right of the household member. If you need to make
a change, select change to the right of the person and alter the self employment
information.
Change Self Employment
This page will allow you to change any kind of self employment information. If you
wish to delete self employment check the box that says 'Check here to delete this
self employment'. You must also provide the date the self employment ended, put
that in the end date text box.
Unearned Income
This page will allow you to add or change unearned income that household members
may have. Typical types of unearned income are, Veteran's Benefits, SSI, Social
Security or Child support. To add any unearned income to a member, click 'Add' to
the right of the member, to change any listed unearned income, click 'change' and
then make the necessary changes.
Unearned Income
This page will allow you to change or delete unearned income that household members
may have. To add any unearned income to a member, put in the amount in whole US
dollars to the right of the type of unearned income. If it's not listed in the types,
put it in other, and then use the drop down to select what type of unearned income
it is. If you select the same type in the other fields, they will just add the 2
same type amounts together.
Income Summary
This page will give you a summary of all the people in the case and their income.
If you need to make a change, select change from the bottom and alter the information.
Expenses
This page will give you a summary of household members and their expenses, if they
have any. If you need to add an expense to the household member, do so by clicking
'Add' to the right of the household member. If you need to make a change, select
change to the right of the person and alter the expense information.
Change or Add Expenses
This page will let you change a household members expenses. Enter the amount in
whole US dollars next to the corresponding expense and click 'Next'.

This page will let you add household expenses. Enter the amount in whole US dollars
next to the corresponding household expense and click 'Next'.
Insurance
This page will show a list of all the insurance companies that are associated with
the household. If you have any it will give you the opportunity to change it. If
none are listed you can click the 'Add' button and add an insurance company to the
household. When you are finished working with the insurance section click 'Next'.
Add Insurance
This page will allow you to select an insurance company that you have coverage through.
If yours is not listed you can select 'other' from the drop down and add the new
policy. When you are done, click 'Next'.
Change Insurance
This page will only allow you to delete insurance. If you need to change insurance
then delete the one on record, if it's no longer applicable, and then add the new
insurance.
Household Resources
This will show a list of resources that we have on file for each person. If you
need to change or delete any listed resource information, select the 'Change' button
next to their name. This will take you to a page where you can change or delete
any existing resource. If you need to add a resource, select 'Add'. When you are
done working with the resource information, select 'Next' at the bottom of the screen.
Resources include assets such as checking or savings accounts, cars, burial policies,
and other.
Change Household Resources
This page will let you change a household resource. Enter the amount in whole US
dollars next to the corresponding resource and click 'Next'.
Real Estate
This page will show the information we have on file for your real estate. You can
make any changes if necessary.
Bought or Sold Resources
This page will show a list of household members, check the people that have bought,
sold, traded, or given away any resources.
Request Benefits page
This page will show a list of benefits you can choose to apply for. If the benefit that you want is greyed out,
then someone else in the household already receives that benefit. You must contact your local OKDHS office
in order to be added to that benefit. There is also a quick eligibility assessment that is run, it will let
you know if you are 'potentially eligible' to receive that SNAP and Medical. Even if the assessment comes
back and says you may not be eligible, you still have the right to apply.
Education and Training
You must provide your Training and Education. Answer if each household member is
currently attending school, also provide their last grade completed and name of
school.
If you currently have TANF on your case, you must state that you read and understand
the information to continue receiving TANF. If you are unable to read the page or
do not understand it, please contact your local OKDHS Human Services Center for
assistance.
Incapacitated
This page will allow you to select the people that are incapacitated.
You can only select people that are incapacitated if someone on the case receives
the TANF benefit. Children do not show up in the incapacitated section.
Immunization
This page will list the immunization levels we have on file for the children listed,
you can change the status if necessary.
You must also select that you understand that you must provide verification of immunization
for children who are not attending school or going to a licensed child care provider,
and that failure to do so will result in a 25% reduction of the TANF payment standard.
Lump Sum
This page will allow you to check that you read and understand the Lump sum policy.
Fleeing Felon
This page will allow you to select any people that are fleeing felons. All new adults
and adults receiving SNAP will show up as a selection.
Child Support Cooperation
This page will allow you to select whether you want to cooperate with child support
or not.
Absent Parent
This page will allow you to select whether there are absent parents or not.
New Absent Parent
This page will allow you to enter a new Absent Parent to your household.
Change Absent Parent
This page will allow you to change Absent Parent information.
Child Care
This page will allow you to view/change the day care that you receive.
New Child Care
This page will allow you to add a day care.
Change Child Care
This page will allow you to change a day care.
Child Care Need
This page will allow you to indicate a need for child care.
Child Care Need
This page will allow you to indicate a new need for child care.
